Community Healthcare Navigator

Job overview

The Community Healthcare Navigator works onsite primarily in senior living communities across the Greater Lansing coverage area, based in East Lansing, MI. Compensation is not specified. CorsoCare Hospice provides hospice and other care services, and this role supports patient education, care navigation, and referral relationships. The navigator follows patients through the healthcare continuum, supports patients and families with education and coaching, and builds relationships with healthcare and senior-living partners. What you'll do

• Promote company service lines and referral business
• follow up with patients during hospitalization and after discharge
• coach and support patients in developing personal healthcare plans
• conduct needs assessments
• facilitate support groups
• provide patient and professional education
• make welcome and follow-up calls to residents starting services
• collaborate with community teams and medical professionals
• build relationships with physicians, hospitals, skilled nursing facilities, senior agencies, and senior communities.
