What you'll do
- Evaluate, diagnose, and treat acute and chronic conditions
- provide preventive care, wellness visits, and screenings
- develop and manage individualized care plans for a predominantly geriatric population
- collaborate with nurse practitioners, care coordinators, and support staff
- document encounters and track outcomes in EHR
- participate in quality improvement
- educate patients and families.
