Case Manager, Supported Employment

What you'll do

• Build trusting, collaborative relationships with clients
• develop individualized employment or education plans
• discuss accommodations, disclosure options, and client rights
• conduct job development and job-search activities, including an average of six employer contacts per week
• provide follow-along support
• coordinate with behavioral health teams, vocational rehabilitation counselors, educational staff, families, and community partners
• facilitate vocational groups
• help clients understand how employment may affect benefits and refer them to benefits counseling as needed
• spend at least 65% of scheduled work hours in the community
• develop and monitor treatment plans and connect clients with resources.
