What you'll do

• Assess patients and establish and evaluate treatment programs, plans of care, goals, and discharge plans in collaboration with physicians
• develop plans with patients or family members
• implement physical therapy treatment plans and select activities to progress patients toward goals
• revise plans with patient or family input
• provide education and training
• provide discharge instructions, follow-up, and referrals as appropriate
• document evaluations, plans, treatment sessions, and required education and discharge information
• observe infection-prevention precautions.
