Hospice Registered Nurse (RN) Case Manager

What you'll do

• Assess patients and collaborate with the care team to develop and implement plans of care
• refer patients to other hospice disciplines as needed or document why a referral is not made
• maintain accurate and timely patient progress records
• record pain and symptom management changes and outcomes
• teach patients, families, and caregivers care techniques
• communicate with the clinical manager and care team
• perform on-call responsibilities and services as assigned.
