The role supports safe, effective treatment through virtual assessments, therapy monitoring and optimization, medication coordination, and quality initiatives.
What you'll do
- Provide medication reconciliation, dose optimization, adherence support, and drug-level assessments
- partner with physicians and interdisciplinary teams to order individualized care plans under a Collaborative Practice Agreement
- monitor therapy effectiveness and safety and intervene as appropriate
- support quality-improvement and REMS initiatives
- assist with complex prior authorizations, workflow improvement, and patient education as needed.
