What you'll do

• Lead, train, supervise, and evaluate Support Coordinators
• oversee targeted case management across three counties
• review and approve Individual Support Plans, assessments, budgets, amendments, and documentation
• guide staff on case management practices and community resources
• coordinate case assignments and transitions
• collaborate with regional partners
• maintain audit documentation
• participate in Individual Support Plan meetings and provide direct coverage when needed
• travel between locations for oversight and staff support
• apply approved crisis intervention techniques according to certification and protocols.

Why this role

Schedule is Monday–Friday, 8:00 a.m.–5:00 p.m. Requires a bachelor’s degree in social work, psychology, counseling, or a related field and at least two years of relevant experience. Must obtain and maintain CPI, MANDT, or CALM certification within 60 days of hire. Travel between locations is required as needed.
