What you'll do

• Serve as a resource on OhioRISE processes and workflows
• monitor member transitions and coordinate follow-through
• coach and mentor staff
• support referral, intake, and engagement timelines
• review enrollment documents and preliminary care plans
• coordinate hospital and residential referrals and transitions
• conduct chart reviews and documentation quality checks
• support compliance and reporting
• and provide limited, temporary care coordination when assigned. Participate in the on-call rotation as assigned.
