What you'll do

• Assist residents and families with transitions into the facility
• complete psychosocial assessments
• identify and address residents’ social needs
• compile social information at admission
• maintain resource lists
• plan, organize, implement, and evaluate social service programs
• assess residents for discharge and coordinate discharges
• advocate for residents
• conduct interviews
• ensure relevant MDS sections and Care Area Assessments are completed in accordance with regulations.
