What you'll do

• Develop, implement, and review resident care plans
• complete quarterly assessments and required MDS portions
• communicate with residents’ families about care changes and updates
• participate in discharge planning and coordinate appropriate resources and referrals
• complete daily documentation, forms, and reports and submit them to the Director.
