What you'll do

• Manage client intake by gathering patient information and assessing service needs
• coordinate caregiver schedules and client coverage
• respond to scheduling inquiries from clients, families, and caregivers
• process referrals and follow up on pending authorizations
• resolve scheduling conflicts, call-offs, and shift changes
• maintain client and caregiver records
• communicate with healthcare professionals, social workers, and referral sources
• contact new referrals within 24 business hours
• meet prospective clients by phone, in the office, or at their homes.
