The Behavioral Health Case Manager is an onsite, full-time position in Panama City, FL, paying $20 per hour. PanCare of Florida’s mission is to bring quality healthcare to underserved communities, and this role supports its Behavioral Health team. The case manager coordinates care for people with behavioral health conditions by connecting patients with providers, family members, and community resources. The role advocates for timely care, supports patients and families, and maintains case records to help improve patient well-being and continuity of care.
What you'll do
- Serve as the primary patient contact and coordinate care across therapists, physicians, and community services
- advocate for timely and appropriate care
- connect patients with therapy, support groups, financial assistance, and housing resources
- provide emotional support, education, and guidance to patients and families
- maintain accurate case records
- connect patients with providers for crisis intervention and stabilization when needed
- collaborate with community organizations.

Why this role
Requires at least one year of practice support experience in behavioral health settings and understanding of behavioral health conditions and treatment methods. Three or more years of relevant experience, Basic Life Support, and crisis intervention training are preferred. Schedule is Monday–Friday, 8:00 AM–5:00 PM.
