What you'll do

• Provide individual, group, and family sessions as indicated
• participate in multidisciplinary team activities and quality-assurance chart reviews
• support discharge and aftercare planning, discharge summaries, clinical documentation, outcome assessments, and clinical notes
• provide counseling in an age-, disability-, and culturally sensitive setting
• and guide clinical staff.

Why this role
