What you'll do

• Consult with patients about medication regimens and collaborate with providers on treatment adjustments
• reconcile medications during transitions of care
• develop and monitor individualized medication plans
• educate patients and support team education
• document plans and progress
• monitor and report adverse medication events
• participate in population health meetings
• conduct chronic care management for Medicare patients
• support medication adherence efforts
• collaborate with population health and case management leadership.
