Client Care Coordinator

What you'll do

• Build relationships with referral sources
• respond to referrals and conduct in-home consultations
• guide intake and onboarding and establish care documentation and emergency procedures
• conduct client check-ins, home visits, and quality reviews
• update care plans
• coordinate caregiver matching with Scheduling
• address client and family questions and concerns
• participate in an assigned after-hours on-call rotation.
