• Assess and monitor patients’ physical, emotional, and psychological needs
• create home health care plans aligned with physician orders and patient goals
• administer medications, treatments, and interventions
• provide pain and symptom management
• educate and support families and caregivers
• collaborate with an interdisciplinary team
• maintain accurate documentation
• participate in the local branch on-call rotation as required.
