What you'll do
- Help eligible participants complete intake and housing applications
- coordinate affordable housing searches and inspections
- support move-ins and weekly home visits
- connect participants with community services and appointments
- maintain case notes, files, service plans, rental calculations, invoices, and HMIS data
- assist with activities of daily living
- and coordinate with landlords, service providers, and the supportive housing team.

Why this role
