Patient Care Coordinator

The Patient Care Coordinator role is onsite in Portland, OR. Compensation is not specified. The three-month assignment, with possible extension, supports inpatient care coordination and requires weekend availability, including Sunday through Saturday and holidays; the RN may float to at least one neighboring location.

What you'll do

• Collaborate with patients, families, and the multidisciplinary care team to support safe, cost-effective inpatient care
• evaluate treatment appropriateness and facilitate changes with the care team
• support and educate nurses and care-team members
• help maintain care standards and protocols
• assess consultation and discharge needs
• ensure patient information is documented and available to the team
• coordinate with hospital departments and monitor resource utilization
• prepare utilization and cost-effectiveness reports as requested.
