What you'll do
- Develop relationships with hospital case managers, discharge planners, social workers, physicians, and other healthcare professionals
- educate referral sources about services, clinical programs, and admissions
- visit hospitals to identify referral opportunities and support discharge planning
- review referrals with clinical and admissions teams
- coordinate transitions to skilled nursing facilities
- track referrals and report activity and performance
- participate in networking and community outreach
- monitor relevant post-acute care guidelines
- meet referral development and census growth goals.
