What you'll do

• Plan, organize, and direct home health services
• conduct initial evaluations and assessments
• manage pain and medications
• develop and revise care plans based on patient needs
• prepare timely clinical notes
• coordinate with PT, OT, ST, MSW, and HHA team members and primary care physicians
• educate patients and caregivers.
