What you'll do

• Complete OASIS assessments and physical assessments
• respond to referrals and complete evaluations within agency timeframes
• develop and update individualized plans of care aligned with physician orders and patient goals
• provide wound care, medication management, IV therapy, and chronic disease management
• educate patients and families
• coordinate with physicians and the interdisciplinary team
• document visits and OASIS accurately and on time
• identify needs for additional services and initiate referrals
• participate in team conferences and quality improvement
• supervise home health aides when assigned.

Why this role
