What you'll do

• Provide evidence-based behavioral health interventions in primary care
• assess referred patients and develop care plans
• use screening and assessment tools
• collaborate with medical providers
• refer patients to specialty and community resources
• document in the electronic health record
• participate in chart reviews, quality improvement, guideline development, staff education, and Patient-Centered Medical Home collaboration
• and guide and supervise the Behavioral Health Case Manager.
