This role exists to provide high-quality, compassionate hospice care outside standard business hours, ensuring seamless coverage and continuity of care for patients and families during nights, weekends, and on-call hours.

What you'll do

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
• Conduct initial and ongoing assessments.
• Support the IDG process through chart reviews and interdisciplinary communication.
• Educate patients and families on disease progression and symptom management.
