Care Transition Case Manager (Registered Nurse) — Remote, Knoxville Area

What you'll do

• Conduct face-to-face member visits at area hospitals and collaborate with hospital teams on discharge planning
• coordinate post-acute care and services
• serve as a liaison among members, providers, and internal or external customers
• oversee complex cases and implement interventions
• perform case management assessment, planning, coordination, monitoring, and evaluation
• support utilization management for complex cases as needed.
