What you'll do

• Develop and manage individualized hospice care plans
• provide nursing assessments, interventions, education, and symptom management
• monitor conditions and adjust plans
• coordinate interdisciplinary care
• collaborate with physicians and other care partners
• delegate and supervise LPNs and hospice aides
• educate and support patients and families
• complete compliant documentation
• monitor medications, supplies, and equipment
• participate in quality initiatives and mentor new team members.
