What you'll do

• Provide nutrition assessments for admissions, quarterly and annual assessments, tube feedings, and weight changes.
• Create and update care plans for nurses and CNAs.
• Complete MDS for Section K and CAAs.
• Use nutrition-focused physical exams to identify malnutrition.
• Collaborate with the interdisciplinary team and conduct monthly kitchen inspections.
