What you'll do

• Assist patients, families, or significant others with social, economic, or environmental problems related to behavioral health diagnoses
• help patients anticipate and overcome barriers in the healthcare system
• support understanding of diagnoses and treatment plans
• coordinate access to appropriate social services
• assist communication of treatment plans with families and caregivers
• document interactions and outcomes
• and partner with community agencies to address psychosocial needs.

Why this role

Requires a bachelor’s degree and at least one year of related behavioral health social work experience. A bachelor’s degree in social work, social work license or certification, MSW or counseling master’s degree, counseling experience, patient navigation or discharge planning experience are preferred; BCLS certification is required.
