Registered Nurse, Home Health

Job overview

The Registered Nurse, Home Health role is onsite in Conrad, MT, and pays $32–$45 per hour. The nurse reports to the Home Health Director and provides services under patient plans of care. The role evaluates and reassesses nursing needs, coordinates care, and delivers specialized, preventive, and rehabilitative nursing services to support patients and their families at home.

What you'll do

• Make initial evaluation visits and regularly reevaluate nursing needs
• initiate and revise plans of care
• provide specialized, preventive, rehabilitative, and total patient care as needed
• prepare clinical and progress notes and care-conference summaries
• coordinate services and inform personnel of changes in patient condition or needs
• counsel patients and families
• process orders and notify physicians
• complete certification, recertification, discharge, and OASIS documentation
• supervise and teach nursing personnel
• participate in care conferences, peer review, and quality assurance
• take on-call duty nights, weekends, and holidays as assigned.
