What you'll do

• Plan and coordinate job-readiness and transition services
• facilitate workshops and coordinate work experiences
• work with service providers, agency staff, host-business staff, and school districts
• implement individual transition plans
• identify and develop work-site rotations
• evaluate participant skills and job matches
• monitor progress and adjust plans
• provide direct training and support
• maintain regulatory compliance and program operations.
