The Registered Nurse (RN) – Outpatient and Community-Based Services role is onsite in Lewiston, ME, with visits in community settings, homes, hospitals, care facilities, and physician offices as participants’ needs require. The nurse supports Spurwink’s interdisciplinary services for adults and children with mental health needs, including serious mental illness and substance use disorders. The RN advances prevention, health promotion, and population health across regional programs by coordinating care, educating participants and families, and integrating health goals into treatment plans. The role contributes nursing expertise to multidisciplinary care and supports participants’ independence in managing their health.
What you'll do
- Engage, assess, monitor, educate, coordinate, and provide interventions for participants and families
- conduct in-person visits
- facilitate group treatment and education
- contribute health information to multidisciplinary teams
- assist with individual service plans and document care
- coordinate referrals and collaborate with community providers
- maintain records and outcome data
- serve as a staff resource
- provide backup medical-assistant support as described.
