What you'll do

• Establish referral relationships and present the home health care model
• advise the agency on community needs and program development
• collaborate on policies for new programs
• ensure marketing compliance with applicable laws
• collaborate with administrative and supervisory personnel
• partner with the Interdisciplinary Group to support patient and family care
• develop public relations and working relationships with physicians and community agencies
• meet or exceed monthly admission goals.
