What you'll do

• Provide case management and service coordination for eligible Veteran-Directed Care clients.
• Advise veterans and caregivers about available services and resources, conduct interviews and assessments, provide options counseling, and develop comprehensive care or service plans.
• Use communication, computer, software, and mobile technology skills in client work.
