What you'll do

• Coordinate care and services for selected patient populations
• review admissions and utilization across payer types
• lead interdisciplinary work toward patient outcomes
• assess discharge needs and barriers early
• communicate with physicians, staff, patients, families, and outside agencies
• maintain and develop case management policies and procedures
• conduct continued-stay reviews using InterQual criteria
• issue notices of non-coverage as required
• educate staff
• lead case management meetings
• develop and report quality-improvement initiatives
• support compliance and patient safety.
