What you'll do

• Provide ACT services, case management, linkage, and advocacy
• assess client needs and develop person-centered treatment plans
• provide individual or group therapy and crisis intervention
• support clients with housing, entitlements, and community appointments
• coordinate with the treatment team
• complete clinical documentation and billing
• participate in monthly 24-hour crisis coverage or as assigned.
