What you'll do

• Conduct in-home assessments of activities of daily living, cognition, mobility, fall risk, home safety, medication self-administration, and dietary needs
• develop, review, and update individualized care plans
• complete visits at required intervals and reassess when needs or conditions change
• document assessments and plans in AxisCare
• communicate client needs and changes to the office team.
