What you'll do

  • Provide psychiatric rehabilitation services at a site or in the community as assigned
  • facilitate employment and community resources and opportunities
  • monitor client progress and participate in individualized rehabilitation plans
  • coordinate vocational opportunities and transitional employment coverage
  • maintain employer and provider networks
  • facilitate daily living, socialization, educational and vocational supports, and leisure activities
  • participate in case conferences and coordinate with community providers.
