Licensed Nurse – Weekend Visit Nurse (LPN)

The Licensed Nurse – Weekend Visit Nurse works onsite in Fort Wayne, Indiana, with visits assigned as close to home as possible. Pay is $27.50–$38.50 per visit. Community Home Health Care and CIHC provide in-home patient care and clinical oversight. The LPN conducts patient visits, assesses clinical needs, coordinates care, and supports quality assurance and patient and caregiver satisfaction. The role also guides clinical staff as needed and helps ensure accurate, timely, compliant documentation for adult and geriatric patients.

What you'll do

• Conduct client visits and participate in quality improvement
• investigate and resolve patient and staff complaints
• maintain accurate, complete, timely, and compliant clinical documentation
• assess patient status and interpret clinical data
• coordinate with patients, families, caregivers, and interdisciplinary teams
• provide oversight and guidance to clinical staff as needed.

Why this role
