OIDD Support Coordinator

What you'll do

• Complete quarterly visits and monthly contacts
• conduct annual CPOCs within funder timelines
• schedule participant and provider meetings
• respond to changes in participant needs and submit paperwork
• research and link services
• coordinate with providers
• manage critical-incident responsibilities
• attend supervision, staff meetings, and training
• support participants during emergencies and evacuations
• fulfill mandated-reporter duties.
