Navigation Services Manager

What you'll do

• Supervise, coach, and support Health Navigators
• conduct individual supervision, team meetings, case reviews, and performance evaluations
• set expectations and monitor productivity, documentation, attendance, follow-up, and service quality
• manage staff schedules and coverage
• review care-panel notes and documentation for quality and compliance
• coordinate program activities and resources
• monitor caseloads, service outcomes, and deliverables
• support complex cases and service barriers
• collaborate with medical, behavioral health, outreach, and housing teams
• train and onboard navigators
• report staffing, performance, risks, and service gaps to leadership.
