What you'll do

• Provide hands-on clinical nursing
• assess and coordinate pain and symptom management with patients, families, physicians, and the interdisciplinary team
• support patient choice, relief of suffering, and dignity
• educate and support families
• participate in interdisciplinary meetings and update plans of care
• coordinate hospice services and referrals, medications, and durable medical equipment
• educate patients and families about disease progression
• assist with continuous quality improvement activities
• report changes in patient condition to the Director of Nursing.
