PRN Hospice Chaplain

What you'll do

• Assess spiritual needs of patients and families or caregivers
• contribute to individualized care plans
• provide direct spiritual care
• liaise with community chaplains and spiritual counselors
• document services and communications
• attend patient-care conferences and interdisciplinary meetings
• provide spiritual-care consultation and education
• recruit community spiritual-care resources
• provide requested funeral or memorial services
• participate in quality improvement.
