Elder Outreach Worker

The Elder Outreach Worker role is onsite in Perronville, MI, and pays $18.50 per hour. The role also coordinates referrals and programs, maintains documentation, and reports status and unusual findings to the Registered Nurse.

What you'll do

• Build relationships with tribal elders and families
• conduct psychosocial assessments and interviews
• provide case management and referrals
• maintain updated files and documentation
• coordinate services supporting aging in place
• follow up on referrals
• organize elder program involvement
• assist with applications, transportation, meals, and educational events
• report status and unusual findings to the Elder Services Registered Nurse.
