What you'll do

• Conduct preliminary home visits and ongoing client assessments
• complete supervisory visits every 30 and 60 days
• update plans of care
• perform home safety checks
• supervise, teach, and orient caregivers
• identify and address caregiver performance issues
• co-manage caregiver training
• participate in quality assurance evaluations
• and travel within the agency territory.
