Mountain View PACE is expanding in Kentucky to bring person-centered care to more older adults; the role supports seniors and families through program enrollment.

What you'll do

• Meet with prospective participants and families to explain the program and assess needs
• complete in-home and community-based assessments
• coordinate evaluations, documentation, and intake steps
• attend intake appointments and support participants through enrollment
• complete enrollment packets
• serve as the main contact for referrals
• present referrals to the Interdisciplinary Team
• maintain CRM and EMR records
• coordinate with the Medicaid Enrollment Specialist
• participate in meetings, training, and quality improvement
• track enrollment goals and metrics
• and help identify and address enrollment barriers.
