Transitions LifeCare’s mission is to support individuals and families through serious illness and end-of-life journeys. The nurse provides hospice visits, admissions, and case-management coverage across patient homes and care facilities. The role helps maintain continuity of care during weekends, urgent needs, staffing absences, and periods of increased census through clinical judgment, care coordination, and support for patients, families, and colleagues.

What you'll do

• Conduct scheduled and PRN hospice visits
• assess patient needs and provide skilled nursing care aligned with the hospice plan
• monitor conditions and implement interventions to maximize comfort and quality of life
• conduct elective, urgent, and emergent admissions
• initiate crisis care under agency protocols and Medicare regulations
• cover RN Case Managers during leave or increased census and function as case manager when assigned
• coordinate disciplines involved in care
• delegate and supervise LPNs and Certified Nursing Assistants
• coordinate equipment, medications, supplies, and contracted services
• participate in quality improvement and assist with onboarding and mentoring.
