Registered Nurse (RN) - Case Manager

Identifies patients at risk for adverse outcomes during transition. Performs comprehensive assessment of psychosocial, medical, and discharge needs. Reassesses patient's clinical condition and develops strategies to mitigate readmission risk. Coordinates the plan of care and discharge plan with the multidisciplinary team.
