What you'll do

• Provide home-based care to patients with illnesses, injuries, and chronic diseases
• work with physicians and oversee LPNs, CNAs, and aides
• assess patients, document progress, and report changes to physicians
• educate patients and families on care plans, medications, therapy, diet, exercise, and home safety
• coordinate goals and discharge from services.
