What you'll do

• Assist individuals with Community Connect, Free Through Recovery, and 1915(i) applications
• support shopping checkout and navigation of local community services
• help clients connect with assigned Care Coordinators and maintain communication
• manage incoming mail and respond to online inquiries
• provide compassionate, client-centered assistance
• document outreach and application support for 1915(i) compliance
• help build and maintain relationships with community resources and partners.
