What you'll do

• Assess patients identified at nutritional risk, including needs related to substance use disorders and recovery
• develop, implement, and monitor individualized nutrition care plans
• provide nutrition education and counseling to clients and families
• collaborate with physicians, nurses, therapists, case managers, and other care team members
• maintain timely electronic medical record documentation
• monitor weight, laboratory results, fluid status, and clinical condition
• participate in quality improvement, clinical rounds, and case conferences
• provide nutrition expertise to facility staff.
