The Enhanced Care Management Care Manager, Registered Nurse role is onsite at the SBC Clinic in San Bernardino, CA. Compensation is $46.20–$60.05 per hour. As part of the Care Management Team, the RN coordinates clinical care for high-risk patients with acute and chronic conditions and medical, behavioral, or social needs. The role develops and reevaluates care plans, coordinates across providers and support services, monitors outcomes and resource use, and provides mentorship to other team members.
What you'll do
- Review electronic medical records and develop, implement, and evaluate individual care plans with providers
- communicate patient needs and recommendations across care teams
- collaborate with clinicians, social services, patients, and families
- support patient activation and evidence-based care
- monitor healthcare resource use and outcomes
- manage acute and chronic conditions
- prioritize cases
- oversee Patient Navigators and Care Coordinators
- support policies, guidelines, and medical-appropriateness documentation.
